Your passport should have a validity for a minimum of 6 months. If you do not have 6 months validity you cannot apply for Umrah visa.
Yes. You must have a valid UAE Residence Visa to apply for Umrah Visa from UAE. The UAE Residence Visa should have a validity for a minimum of 3 months.
Those with Visit or Tourist Visa can travel for Umrah from UAE but cannot apply for Umrah Visa from here. They will have to apply and get the Umrah Visa from the country of their residence or native country.

Women and children traveling to perform Umrah must be accompanied by a Mahram (Husband / Father / Brother). A relationship certificate is required if the Mahram is father or brother. Women over 45 years of age do not require a Mahram and can travel alone to perform Umrah.

No. You can reach Mecca by flight or by bus. If you are taking the flight, you can enter KSA through Jeddah or Madina.
